Complex care at home

What is complex care at home?

Complex care is for people whose health needs go beyond everyday personal care. This might be because of a neurological condition, a brain or spinal injury, breathing support, feeding through a tube, or several conditions at once.

Carers are trained for the specific tasks in your parent’s care plan, and are often supported by nurses employed by the agency. Other people such as the GP, district nurses and specialist teams may also be involved.

The agency will do a detailed assessment and agree a care plan with your parent, the family and the health professionals involved. It can be visiting care, overnight care or around the clock, depending on need.

What carers can help with

Every care plan is different. These are the things carers most often help with.

  • Carry out specialist tasks they are trained for, as set out in the care plan
  • Help with personal care, moving and positioning
  • Use equipment such as hoists safely
  • Follow routines agreed with nurses and specialist teams
  • Prompt or give medicines, as agreed in the care plan
  • Keep careful records and report changes
  • Support your parent to take part in daily life and things they enjoy

How it’s arranged

What usually happens once an agency gets in touch.

01

First steps

The agency will carry out a detailed assessment, often involving a nurse, and will want to speak with the health professionals who know your parent. Discharge from hospital may need careful planning, so involve the agency early.

02

Planning the care

They will write a care plan and build a team of carers trained in your parent’s specific needs. Ask how they train carers, who supervises them, and whether you can meet the team.

03

Once care begins

Once care starts, the plan is reviewed regularly with the family and health team. Ask how the agency handles emergencies and how they keep you informed.

Costs and funding

What does complex care at home cost?

The cost depends on the level of skill and the number of hours needed. Our cost guide explains what affects the price, and the calculator helps you estimate it. Care may be paid for from savings, by the council after a needs and financial assessment, or by the NHS in some cases, and people with complex health needs may be eligible for NHS Continuing Healthcare. Ask the GP or hospital team about an assessment.

Questions families ask

What counts as complex care?

It usually means health needs that require specialist training, such as tube feeding, breathing support or care after a serious injury. The agency will assess whether your parent’s needs fit.

Are complex care carers nurses?

Not always. Many are care workers trained in specific tasks and supervised by nurses. Ask each agency how their team is set up.

Could the NHS pay for complex care?

Possibly. People with complex health needs may be eligible for NHS Continuing Healthcare, so ask the GP or hospital team about an assessment.

Can complex care start straight from hospital?

It can, but it needs planning. Involve the agency and the hospital discharge team as early as you can.
